consists of two separate and distinct case management roles:.5 FTE
Housing Case Manager and.5FTE Medical Case Management. for a full
time, non-exempt, union, regular position. The Housing Case Manager
partners with clients within the office setting and within the
client's home to deliver services which focus on accessing
temporary and long-term housing, improving housing stability
through life skills development, and acting as a liaison between
clients and landlords. The Medical Case Manager assists clients
with chronic conditions to access and engage in medical care,
treatment, mental health, substance use services, as well as other
community services which support the individual's wellbeing and
reduce barriers to care. Essential Duties Engages and builds report
with clients, completes assessments, and co-creates service plans
to address client's needs which are implemented, monitored, and
revised regularly. Develops a working knowledge and collaborates
with community resources to link clients with a variety of public
and private programs. Navigates complex health care and insurance
programs; coordinating with multi-disciplinary teams and clients to
reduce barriers to care and prevent gaps in health coverage.
Assists clients with housing searches, advocates with
landlords/property managers, and addresses barriers to housing both
short and long-term housing solutions. Provide support and
education on the Landlord/Tenant Laws and advocate for
participant's rights as tenants, respond to complaints and develop
positive liaison relationships with landlords and housing
authorities. Build awareness of HIV in and effort to increase
community knowledge and reduce stigma. Maintains timely, accurate
and confidential client files, in accordance with departmental
standards. Core Competencies Successful experience working with
individuals who experience complex chronic health conditions,
behavioral health barriers, chemical dependency, poverty,
homelessness, and stigma-induced isolation. Comfortable working
with individuals along the entire continuum of diversity; including
but not limited to varying abilities, race, class, gender identity,
and sexual orientation. Clear understanding and adherence to
professional boundaries, ethics, HIPPA principles, and self-care
practices within a trauma-informed environment. Well-developed
ability to build strong working relationships with colleagues,
external providers, and partner agencies. Demonstrated clear,
professional, and engaging communication in both verbal and written
form. Comfortable working independently, with accountability, sound
judgment, discretion, and professionalism. Demonstrated ability to
organize, prioritize multiple projects, and meet deadlines in a
time-sensitive environment. Ability to learn, accept feedback and
adapt to ongoing client and programmatic changes; ability to
respond quickly to change. Strong working knowledge of Microsoft
Word, Excel, web-based meeting platforms and experienced in
database entry with attention to detail. Education and Experience:
Three years professional experience providing housing or case
management (or similar) services to people with high barriers to
care. Preferred Bachelor's degree in a social services or related
field, equivalent experience is acceptable. Lived experience:
individuals from non-dominant cultures encouraged to apply. Cover

& Work Environment Standard office environment: the position is in
an office setting, with multiple meetings at community agencies and
within client's homes. Office location in Bellingham with travel
primarily in Whatcom and Skagit Counties with periodic travel in
San Juan and Island Counties. All staff are currently required to
use personal protective equipment to prevent exposure to COVID-19,
and proof of full COVID-19 vaccination is required as a condition
of employment. Qualified candidates must possess a valid driver's
license to access either a personal vehicle or an agency-provided
vehicle; mileage will be reimbursed at the standard IRS rate.
