experience (Med Surg, LLOS, ED) - Candidates must be willing to
work every other weekend - Candidates must float between like units
Description: This position involves working as a case manager
focused on care coordination and discharge planning within an acute
care facility. The ideal candidate will possess experience in
discharge planning and care coordination. Strong candidates with a
background in diverse case management areas are preferred.
Candidates will work across all areas of the facility. Behavioral
health experience does not count as acute care hospital experience.
All travelers are expected to float between various units within
their skill set. Candidates must commit to working three of the
five major holidays (Thanksgiving, Christmas Eve, Christmas Day,
New Year's Eve, New Year's Day). No local candidates will be
accepted for this position. Candidates must submit all requested
time off at the time of application. References are required to
verify recent experience. Successful candidates will be provided
with communication devices needed to perform their duties. Any
expenses incurred for work-related communication must be submitted
for reimbursement. Modules are non-billable and must be completed
