Company: Providence at Home with Compassus Position Summary Make
a lasting impact by joining Providence at Home with Compassus ,
leaders in home-based hospice and palliative care. We’re seeking a
compassionate and experienced Nurse Practitioner to provide
essential support to patients and families across various care
settings, including homes, long-term care facilities, and
hospitals. The Hospice and Palliative Care Nurse Practitioner (NP)
functions independently as an advanced nursing professional and has
responsibility to coordinate and assess care needs, create and
modify the plan of care, make appropriate referrals for patients in
collaboration with the hospice and palliative care
interdisciplinary team. The Nurse Practitioner works in conjunction
with the interdisciplinary team to complete all regulatory
requirements such as provider face 2 face , end of life visits, and
assessments for re-certification. The NP, under direct physician
order or by physician approved protocols and within their scope of
practice may independently order opioids, adjusting dosages and
change other drugs, as patient condition requires. Other
responsibilities include on call coverage rotation with the other
providers. In addition to the clinical duties, the NP is a primary
link and resource to patients, families, and staff for education
related to hospice and palliative care. They maintain schedules and
productivity standards, document and maintain data on all patients
referred and seen, and participate in quality improvement
activities as it relates to hospice and palliative care. Position
Specific Responsibilities • Complies with organization-wide
specific policies and procedures, including working knowledge of
policies, reference to policies as needed and consistent compliance
with same. • Complies with patient care requirements of all
regulatory agencies including working knowledge of policies as
needed and compliance with same. • Actively creates an environment
of safe delivery of patient care and functions with a constant
awareness of safety issues identified including immediately
correcting/reporting any unsafe conditions found. • Under required
physician supervision independently conducts a thorough medical,
functional, and psychosocial assessment on assigned patients;
performs and interprets diagnostic tests and procedures as defined
by protocol; assesses whether patient’s condition requires the
immediate attention of the physician. • Provides appropriate
medical intervention when needed, initiates and alters certain drug
therapies, and writes orders according to established protocols
approved by the physician or direct physician orders. • Performs
Face 2 Face, End of Life, and recertification assessment visits. •
Collaborates with other members of the health team in providing
continuity of care to meet the unique needs of the patient;
provides patient information to the physician. • Assists the
physician by ordering drugs and supplies, keeping records, as
defined by protocol. • Participates in documenting notes in the
clinical medical record. • Participates in interdisciplinary team
meetings, recommends course of treatment and monitors ongoing care.
• Provides liaison and information to physicians, discharge
planning, and other departments regarding quality of care issues. •
Recommends appropriate action based on the patient’s condition. •
Under direct physician order or by physician approved protocols
independently order IV therapy, including antibiotics and opioids,
altering dosages and changing to other drugs, as patient condition
requires. • Furnishes medications per standardized procedures
including medications available under Providence St. • Josephs
Health formulary or those medications usually prescribed by
specialty physicians. • Ordering feedings and placement/replacement
of nasogastric or replacement of gastric tube, adjusting rate and
formula based on patient’s needs, as determined by nutritional need
and/or swallowing capabilities. • Assists family and patients
regarding decisions including goals and values, withdrawal and
withholding of interventions, non-beneficial interventions and
resuscitation status. • Educates the patient and family regarding
diagnosis of medical condition and prognosis. • Assists in the
referral of patients to appropriate health facilities and community
resources. • Provides education to nursing staff/nursing facility
departments, and family members regarding the special needs of the
patient and elderly population. • Establishes and maintains
courteous cooperative relations with the public, patients and other
personnel. • Participates in the development and implementation of
education based on evidence based practices. • Performs other
duties as . • Demonstrates working knowledge of the Hospital
Information System. • Performs documentation in patient’s chart
which is timely , accurate and concise per organization
policies/procedures and regulations. • Utilizes computer based
communications and data base system. • Participate in Provider on
call rotation. • Performs other duties as assigned. Education
and/or Experience Required - Master's Degree Nursing Required - 2
years Acute or transitional care setting Certifications, Licenses,
and Registrations Required - An active and unencumbered RN license
in the state of practice. Required upon hire: National Provider BLS
- American Heart Association (Vendor Managed) Physical Demands and
Work Environment: The demands of this role necessitate a team
member to effectively perform essential functions. Adaptations can
be made to accommodate team members with disabilities. Regular
standing, walking, and manual dexterity are fundamental, along with
the ability to lift and move objects up to 50 pounds. Visual acuity
requirements include close and distance vision, color and
peripheral vision, depth perception, and the ability to adjust
focus. In a healthcare setting, exposure to bodily fluids,
infectious diseases, and conditions typical to the field is
expected. Routine use of standard medical equipment and tools
associated with clinical care is essential. This description

$73.67 The amounts listed are the base pay range; additional
compensation may be available for this role, such as shift
differentials, standby/on-call, overtime, premiums, extra shift
